Аденоидная ткань, или третья миндалина, есть только у детей. Она развивается примерно к 3—6 месяцам, после 12 лет начинает постепенно уменьшаться, к 16—20 годам становится почти незаметной, а в более старшем возрасте уже полностью отсутствует.

Какая же роль предназначена природой этой миндалине? Третья миндалина, входя в глоточное кольцо, создает дополнительную преграду для болезнетворных агентов, фильтруя воздух, вдыхаемый через нос и защищая тем самым гортань, бронхи и легкие.

Но это только до тех пор, пока ее размеры остаются нормальными. Под влиянием частых инфекционных заболеваний, вызывающих воспалительную реакцию лимфоидной ткани, третья миндалина может разрастись и закрыть вход в носовую полость. Вот тогда-то ребенок начинает дышать ртом. Разросшаяся миндалина не только перестает быть барьером на пути возбудителей респираторных заболеваний, она, наоборот, провоцирует их, так как в ее складках и бороздах начинают скапливаться болезнетворные микробы. Такой очаг тлеющей инфекции ежеминутно грозит пожаром— и действительно, спутником аденоидов становятся ангина, трахеит, бронхит. У ребенка часто болят уши, его преследует упорный насморк.

Длительное нарушение носового дыхания ухудшает снабжение организма кислородом, а это сказывается не только на самочувствии, но даже на успеваемости ребенка: он становится рассеянным, невнимательным. К тому же, разрастаясь, аденоидная ткань нередко закупоривает и слуховую трубу, из-за этого нарушается вентиляция среднего уха и снижается слух, может появиться гноетечение из уха.

У ребенка, который постоянно дышит ртом, неправильно развивается лицевой скелет: нижняя челюсть отвисает, верхняя как бы сдавливается, что нарушает процесс формирования зубов. Верхние резцы начинают выступать вперед.

И мало кто знает, что даже развитие грудной клетки, даже изгибы позвоночника могут изменяться в зависимости от неправильного дыхания. Вот как далеко простирается отрицательное влияние разросшейся третьей миндалины!

Конечно, все эти изменения возникают не сразу. И ясно, что чем раньше будет оказана ребенку помощь, тем вернее можно их предупредить.

Как правило, лечение бывает хирургическим — разросшиеся аденоиды удаляют. Услышав от врача, что нужна операция, родители обычно огорчаются. И всячески стараются отдалить неприятный момент, когда надо вести ребенка к отоларингологу. А выжидание идет только во вред.

Но, как и каждая операция, удаление аденоидов требует подготовки. Надо побывать с ребенком у стоматолога и если есть больные зубы, обязательно вылечить их. Предварительно назначают исследование крови на свертываемость. Если она понижена, то врач обычно выписывает витамин К в таблетках. Принимают его в течение трех дней по одной таблетке три раза в день.

После операции ребенку надо на 3—4 дня создать постельный или хотя бы полупостельный режим. Следите, чтобы он не перегревался, не давайте ему горячего чая, горячего молока. Пища должна быть холодной или теплой. Первые дни пусть ребенок ест каши, кисели, овощные и фруктовые пюре, молотое мясо. Некоторые дети и после операции уже не по необходимости, а по привычке продолжают дышать ртом. За этим надо последить.

Третья миндалина не имеет капсулы, она не резко отграничена, а дает языкообразные выросты в подлежащую ткань. Поэтому полное удаление аденоидов невозможно, и у некоторых детей со временем они вырастают вновь. Для предупреждения таких рецидивов очень полезно до операции и после нее провести курс противовоспалительного лечения. Врач обычно назначает орошение носоглотки различными лекарственными препаратами. Его можно проводить и дома, воспользовавшись специальным ручным распылителем для жидких лекарственных веществ. Такие распылители продаются в аптеках. Пользоваться ими просто. Не забывайте только подогреть лекарство.

Противовоспалительное лечение не заменяет операции. Лишь в тех случаях, пока аденоиды увеличены мало и не оказывают отрицательного действия на организм (или есть противопоказания к операции), врач может ограничиться таким лечением.

Показания к операции определяются в зависимости не от возраста, а от состояния ребенка. Иногда аденоиды приходится удалять даже у грудных детей, ибо для них разрастание аденоидной ткани особенно мучительно.

Многие спрашивают: есть ли какие-то новые, более эффективные методы лечения? Пока можно говорить о первых опытах применения лазера, о попытках облучения рентгеновскими лучами, вызывающими сморщивание аденоидов. Но эти методы требуют еще дальнейшего совершенствования.

Сегодня самым безопасным и самым надежным остается хирургическое удаление аденоидов. Если вашему ребенку назначена такая операция— не советую медлить!
